Timely evidence when the plan needs to move forward

Assessment and report are completed within two weeks once the referral is confirmed, the required documents are received and the assessment has taken place. We will tell you promptly if the referral is not clinically suitable or the timeframe cannot be met.

Functional Capacity Assessments Direct is led by Andrea Christian, Senior Occupational Therapist. The service brings a focused functional lens to complex presentations, including neurological and psychosocial disability.

The aim is simple: understand how disability affects the person’s everyday life, identify the support needed for safety and participation, and communicate the evidence in a report that is structured, specific and useful.

Who can use this service?
The service currently accepts plan-managed and self-managed NDIS participants. Referrals are reviewed for clinical suitability before an assessment is booked.
What does “completed within two weeks” mean?
The two-week timeframe begins once the referral is confirmed, required documents have been received and the assessment has taken place. It remains subject to clinical suitability and appointment attendance. Any issue affecting the agreed timeframe will be communicated promptly.
What information is helpful at referral?
A brief description of the participant’s goals, diagnosis or disability, reason for the assessment, funding type, plan dates and any important communication or safety needs is helpful. Existing reports may be requested after a secure method of sharing documents has been agreed.
Are standardised assessments used?
Standardised measures may be used where they are clinically appropriate and meaningful for the participant. The specific assessment approach is selected according to the referral question and functional presentation.
Can a support person attend?
Yes. Participants may involve a family member, support worker, support coordinator or other trusted person. Their role and consent to share information will be discussed as part of the assessment.
How are fees confirmed?
A service estimate and agreement are provided before the assessment is booked. The scope and likely assessment time are discussed so that costs are clear in advance.
